Please download, print and return the completed Registration Form to the Secretariat with appropriate fees.  Photocopy of the form may be used.  Registration will only be confirmed on receipt of full payment.

Replacement Delegates

All delegates would be issued registration numbers in the acknowledgement letters.  Delegate who replaces is required to bring along the acknowledgement letter.

Hospital-Sponsored Delegates

Please submit LPO with registration form; otherwise, a letter of undertaking from the hospital is required.

Payment

Cheque/bankdraft is to be made payable to "Intensive Care Section, MSA".

Cancellation & Refund Policy

The Conference Secretariat must be notified in writing of all cancellations.  Refund will be made after the Conference as follows :

Cancellation on or before 16 June 2008 : 50% refund

Cancellation after 16 June 2008 : No refund

Certificate of Attendance

Certificates of attendance will be issued to all delegates.

Liability

The Organising Committee will not be liable for personal accidents, loss or damage to private properties of delegates during the Conference.  Participants should make their own arrangements with respect to personal insurance.
